- Income Certificate (2024 – 25)
- Online Submission Form (as per applied quota, received at the time of completion of registration)
- NEET Admit Card (with photograph, thumb and duly signed by candidate and parent)
- Passport size 05 Photos with white background (Hard copy & soft copy)

- Caste Certificate (bearing number matching with Caste Validity certificate) as per applied quota & in prescribed format for AIQ/ State – Institutional Quota (Refer NEET 2025 & AACCC 2025 brochure)
- Caste Validity Certificate (bearing number matching with caste certificate)
